About Our Care Navigation Services

Pandora Health, Inc. ("Pandora Health," "we," "us," or "our") provides care navigation services ("Care Navigation Services") which deliver health insights to you to help deliver personalized tips, guidance and support for your health needs. These services include symptom tracking via our AI Coach and consultations with our Gut Gurus and Health Coaches.

The Care Navigation Services are not a substitute for professional advice from a physician or other qualified health care professional. You should always consult with your physician or other qualified health care professional before changing any treatment plan or taking any action that may affect your health.

Our Care Navigators may hold various licensures, including as physicians, nurses, nurse practitioners, dietitians, nutritionists, athletic trainers, exercise physiologists, professional counselors, mental health counselors, or social workers. However, Care Navigators are not acting within the scope of any licensure held when offering the Care Navigation Services.

Scope of Services

The Care Navigation Services do not involve the practice of medicine, nursing, dietetics, nutrition, athletic training, exercise physiology, professional counseling, mental health counseling, or social work.

Our Care Navigators do not and cannot provide diagnosis, treatment, operations, or prescriptions for any illness, disease, injury, ailment, or physical or mental condition. The Care Navigation Services explicitly exclude these medical services.

Your Acknowledgments

By accepting this Informed Consent, you acknowledge and understand that:

  1. Technical Risks: There are potential risks associated with the use of telehealth technology, including but not limited to, technical difficulties that may result from hardware, software, or connection failures that are outside of Pandora Health's control.
  2. Limited Scope: This consent applies only to the Care Navigation Services described herein and not to any other services you may receive from licensed healthcare providers.
  3. Right to Withdraw: You may withdraw your consent at any time without affecting your right to future care or treatment, and without risking the loss or withdrawal of any program benefits to which you would otherwise be entitled.
  4. No Medical Treatment: The Care Navigators do not deliver medical treatment, make diagnoses, or provide prescriptions. These services are for navigation and support purposes only.
  5. Supplement Recommendations: Any supplement recommendations are offered for your convenience. You should discuss all supplements with your healthcare provider before taking them.
  6. Freedom to Purchase Elsewhere: You have the right to obtain medications, supplements, and other products from any pharmacy or retailer of your choice.
  7. Alternative Services: Alternative services, such as in-person consultations with healthcare providers, are available to you.
  8. Appropriate Conduct: Pandora Health staff may terminate a session if you engage in inappropriate behavior, which includes but is not limited to abusive language, harassment, or threats.
  9. No Guaranteed Results: While there are potential benefits to the Care Navigation Services, including improved wellness and health insights, no specific results or outcomes are guaranteed.
  10. AI and Automated Tools: Pandora Health may use AI or other automated decision-making tools as part of the Care Navigation Services to provide personalized recommendations and insights.
  11. Information Sharing: Your healthcare information may be shared with authorized personnel for scheduling, billing, and coordination purposes. Non-essential personnel may be present during virtual sessions for training or quality assurance purposes.

By using our Care Navigation Services, you confirm that you have read this document carefully, understand the risks and benefits of the Care Navigation Services, have had your questions regarding the Care Navigation Services explained, and hereby give your informed consent to receive the Care Navigation Services as described herein.
